What our adult social care mental health teams do
We seek to improve the well-being of adults facing mental health challenges.
We take a preventative approach to personalised support. The main focus of our work is on individuals with care and support needs under the Care Act 2014 and/or with Section 117 Aftercare Needs under the Mental Health Act 1983.Free aftercare under Section 117 of the Mental Health Act
Most individuals we work with are receiving support from a community mental health team (CMHT) but this is not a requirement for referral to the teams.

Carer support
A mental health carer provides unpaid support and care for a person experiencing mental health issues. This role can be taken on by family members, friends, or other close individuals.
Mental health carers assist with various tasks, including emotional support, managing appointments, medication, and daily activities.
Support and resources are available for carers to help them manage their responsibilities and maintain their own health.
