Personal information about you from others
To help us provide services appropriate to your needs we may receive personal information about you from others for example NHS agencies such as GPs, hospitals and CMHT.

The following organisations or departments may use the information you provide:
- in-house occupational health team
- external occupational health provider
- external counsellors; employee wellbeing advisers

What might your information be used for and what is the lawful basis for requesting and using it?
Your information will be used to provide competent and clinically appropriate advice/guidance to the employer and/or employee.

What is the lawful basis for us to use your information?
In the event we commission a GP report for the purposes of a pre-employment medical or management of attendance referral, we will be doing so under GDPR article 6 (1)(e) and Access to Medical Reports Act 1998.

- NHS Dorset – community and mental health services
- Optima Health
- NHS agencies – primarily Dorset based GP’s
- freelance counsellors
