After your assessment your care options will vary depending on your care and support needs, how they will be met and what services you will receive.
Your local authority will work with you to ensure the services you receive are having a positive outcome.
Short-term care
Short-term care is a time-limited support that is used to help people on the road to recovery following an accident, illness, or surgery so that they can live independently again.
Short-term care can be provided in your own home or in a supported living setting.
Long-term care
If you have eligible care and support needs, your adult social care practitioner will work with you to develop a care and support plan. Your plan will focus on what is important to you and will make the most of what you can do, with the help you have around you.
It will outline different aspects of your care needs, including what you can do for yourself, any help you receive from family or friends, your access to community facilities or support from voluntary services, suggestions for self-help equipment or adaptations to your home.
Your local authority will help you explore the care and support options in your area to ensure that you consistently receive the right level of long-term care.
Your care and support plan will be reviewed regularly to see what's working and not working and if it's still the best support for you.
Accommodation
Your local authority can provide information on a range of housing and support options to help you continue living independently for as long as possible. This includes more specialist accommodation such as supported living placements.
