Active 4 Health is a health referral scheme that supports you to increase your physical activity levels.
If you are living with a health condition, you can ask your local health professional to refer you to us. We will match you with activities to suit your location, interest and health needs.
With hundreds of opportunities available each week, both indoors and out, there is plenty of support available to help you make the first steps.

We work with external providers of exercise referral including:
- BH Live
- Everyone Active
- Ferndown Leisure Centre
- Riversmeet Leisure Centre, Gillingham

How to be referred
If you have a health condition and would like to get more active, request an Active 4 Health exercise referral from your GP, Physiotherapist or other health professional.
Contact us if you are unsure if you can be referred, or have any other questions.
Once we've received your referral, we will contact you to complete an initial lifestyle screening. This helps you get started on your active journey.
After your screening, we will book you in for a consultation or an activity from one of the following programmes:
Coast and greenspace
A range of outdoor activities taking place in your local greenspace, suitable for low to moderate risk patients.
Leisure centre activities
A range of activities in your local leisure centre, suitable for low to moderate risk patients.
12 week exercise programme
A 12 week exercise programme with an exercise referral specialist, tailored for moderate to high risk patients.
