Assessing your needs - supported assessment

A supported assessment gives you the chance to tell us about any difficulties you may be having, and what help you think you need. We can look at the kind of support you need to live your life as independently as possible.

The assessment is carried out at your home by a social care worker or occupational therapist. 

To find out more about you we will ask you things such as:

  • how you look after yourself
  • what is working well for you
  • what you would like to change
  • the kind of support you have now
  • your physical and emotional health
  • being part of the community
  • whether or not you have a carer
  • what you have difficulty doing
  • what help you need

Advocacy could be useful if you need help to express your views and wishes.


As part of the assessment we will ask you about anyone who provides you with care and support. This could be your partner, family, friends or even neighbours. You may want them to be with you at the assessment, so that they can offer their ideas and opinions about your care and support. We will also want to tell them about support that is available to help them care for you.

What happens next

Contact us to arrange a supported assessment if you think you meet the eligibility criteria  AND the criteria for financial help.

Once completed, if we agree that you are eligible for help, you will be provided with an estimated care budget. This will tell you how much we will contribute to your care and how much your own contribution will need to be.

Care and support planning

We will then develop a support plan with you. This helps you to think about how you want to use your estimated care budget to get the care and support you need.

Personal budget

 A personal budget will be set once your support plan has been agreed.
